2. How to use this document / tool kit.
The questions contained in this document are questions you should ask your employer reference your school district’s implementation of Executive Order 2021-20 mandating COVID 19 vaccinations and/or COVID 19 testing in your school district. These questions should be sent to human resources in addition to your bargaining unit (Union) if applicable. We would also encourage you to contact your bargaining unit (Union) and advise them not to negotiate your health rights away. The questions in this document are not all-inclusive, and we encourage you to ask any additional questions you may have.
3. Example Letter / Questions
It is my understanding that Governor J.B. Pritzker signed Executive Order 2021-20 mandating COVID 19 vaccinations and/or COVID 19 testing as a condition of my continued employment and/or ability to continue to perform in my role as an educator for (NAME OF SCHOOL). I also understand this is a non-binding resolution and places the burden of enforcement on my employer as the agent in charge.
Pursuant to this request, as my employer, I have the following questions:
Will (NAME OF SCHOOL) provide a physician to discuss the risk/benefits of the COVID 19 vaccination and/or COVID 19 testing, including the assumption of medical liability for adverse reactions?
It is my understanding if an employer requires any medical intervention, it is their duty to provide an occupational medical screening. Will the employer provide an occupational health screening with access to a medical review officer? ……..
